Moppets Registration - IMPORTANT!

To register your child(ren) for Moppets, please send the following information to Stacia Jones (stacia@etownmops.com) before 13 Sep, 2010.

1. Your full name (first, last)
2. Child(ren)'s full name (first, last) and gender (please list all children who will be in Moppets)
3. Child(ren)'s birth year only
4. Any special needs/instructions for each child - please include any known allergies
5. Your address & phone # if different from your MOPS registration one
6. Father's full name (first, last)
7. Please let us know whether the child(ren)'s father shares the same household with you
8. Father's phone # if different from yours
9. Name, address, & phone # of your child's doctor
10. Name & phone # of an emergency contact person - please state person's relationship to child(ren)
